This video shows a nasty bug where an item you put in your inventory will be deleted 30 minutes later. This is probably the reason many of us have seen items disappear. Kittensoft even had a gun despawn right out of her hands.
Bottom line: Do not drag items from containers on the ground (tents, backpacks, clothes) onto the ground. Put the item in your inventory first and then move it to the ground.
